 Questions in section 4.2 re: equipment should have sub-questions (ie equipment) on the same page.   That means that the main question does not need a yes/no option box but is a label only. 

Further Information
The question should be “Select the equipment that is used to support the individual with off bed tasks”
Consider similar wording in Section 3.2 re: equipment

Tony RyanNo presence information (19/01/2009 4:35): Can do, though main question is handy to process procedures etc, much quicker (to code and to run) to check one field than a positive response in any of the other fields, as equipment is prolific in action plan.
